Every athlete MUST have the following forms on file in the Bound app prior to participating in tryouts/practices:
KSHSAA Pre-Participation Physical Evaluation Form (dated after April 1, 2026)
KSHSAA Concussion Release Form
KMC Parental Consent Form for Athletics
