Transcript Request

To obtain an official copy of your Mount Carmel Academy, Chaplain Kapaun Memorial or Kapaun Mt. Carmel transcript, you must provide a signed request for release of records to the registrar’s office at Kapaun Mt. Carmel Catholic High School.  You may choose to fill out a request form by printing and mailing it to KMC, or by filling it out online.  Please choose using either link below to fill out the request form.

You may also choose to send a signed letter which should indicate your name at the time you graduated (maiden name), the year of graduation/withdrawal from MCA, CKM or KMC, the address(es) to which you would like your transcript sent and your current contact information.

Please include the $5.00 per transcript fee for each request with the letter/form, in cash, money order, or check, made payable to Kapaun Mt. Carmel Catholic High School.  Payment must be received before transcript can be mailed.
